Small Group (1-100 full time equivalent employees)
- Download to your desktop the RBG Small Group Request for Proposal Form – this form will prompt you for required information to quote requested medical and ancillary lines of coverage.
- Census (in Excel) – we recommend using the RBG Census Template – inclusive for all lines of coverage. Small group medical requires dependent census information in order to quote dependent coverage.
- Current carrier renewal and most recent invoice for all lines of coverage requested
- Plan summaries for current Ancillary Plans and Medical, if currently on a non-ACA small group medical plan (i.e. Large Group, PEO, Self Funding, etc).
Additionally, Rogers Benefit Group provides agents with complimentary access to the EBIX/Health Connect quoting system. Contact your RBG office at the link listed below for username and password information.
Large Group (101+ full time equivalent employees)
- Download to your desktop the RBG Large Group Request for Proposal Form – this form will prompt you for information required for large group carriers to quote requested medical and ancillary lines of coverage.
- Census (in Excel) – we recommend using the RBG Census Template – inclusive for all lines of coverage. Note – most Large Group medical carriers are now requesting and/or requiring a member-level census to include enrolled dependents first & last names, DOB, gender and zip code.
- Current carrier renewals (if available).
- Most recent invoice for all lines of coverage requested.
- Benefit summaries for all current plans for all lines of coverage requested.
