  • No DE9C Requirement Promo: No DE9C required for groups of 6+ enrolling; virgin groups do not qualify; prior carrier invoice is not required.
  • Reduced Participation Requirement of 25%: Groups with 5+ enrolled are eligible for the reduced participation requirement guideline of 25%. Groups with four or fewer enrolling must meet the 60% participation requirement.
  • New Business Submission Deadlines: 5th of the month following the requested effective date.
