For small group contract terminations, UHC requires a completed Group Termination Request Form. This form is in lieu of a letter on the employer group’s company letterhead. The form must be submitted within 30 days of the requested effective date of cancellation. If the form is received after the 30 days, termination date will be processed first of the month within 30 days. Completed forms should be emailed to clientserviceoperations@uhc.com.
